High-Quality Care

We offer vast care options, including in-hospital support, hospital-to-home, disability care, palliative care, dementia care, specialist nursing services, sleepover and active-night care, live-in home care, and preventative care.

In-Hospital Support: Our team monitors vital signs, administering medication, offering emotional support, and facilitating communication between individuals, their family, and the medical staff.

Hospital to Home: Hospital to Home services focus on ensuring a smooth transition for individuals who are being discharged from the hospital and returning to their homes. Helping Hands Connect coordinate transportation, arranging for any necessary medical equipment or supplies, and providing follow-up care to promote a safe and comfortable recovery at home.

Disability Care: Professional support and assistance for individuals with physical or cognitive disabilities. Our services are designed to enhance their quality of life and independence by providing help with personal care, mobility, household tasks, and social engagement, among other needs specific to their disability.

Palliative Care: We provide a holistic approach to comfort and support for individuals with serious illnesses. Palliative care focuses on relieving pain, managing symptoms, addressing emotional and spiritual needs, and offering assistance to individuals and their families in making informed decisions about their care, with the aim of improving their overall quality of life.

Dementia Care: Catered to individuals living with various forms of dementia, such as Alzheimer's. We offer specialised support and assistance to help manage the challenges associated with memory loss, cognitive decline, behavioural changes, and daily activities. We help with memory stimulation exercises, emotional support, and a safe environment to enhance the well-being of those affected.

Specialist Nursing Services: Our Specialist Nursing service involves the provision of highly skilled and specialised nursing care for individuals with complex medical conditions or specific healthcare needs. These services are delivered by trained nurses with expertise in areas such as wound care, diabetes management, post-surgical care, palliative care, and other specialised fields.

Sleepover and Active Night Care: Caring Hands Connect provide overnight support and supervision for individuals who require assistance during the night. Our caregivers stay awake and alert to address any needs that may arise, ensuring safety, comfort, and reassurance throughout the night.

Live-in Home Care: We provide around-the-clock support and assistance to individuals who wish to remain in the comfort of their own homes. Our team assist with personal care, household help, medication management, companionship, and other necessary support to help maintain independence and a high quality of life.

Preventative Care: Preventative Care focuses on maintaining and promoting good health, preventing the onset or progression of illnesses, and reducing the risk of complications. This includes regular check-ups, health screenings, vaccinations, health education, lifestyle advice, and other proactive measures to help individuals maintain their well-being and prevent the development of avoidable health conditions.

In-Home Help

Our in-home help includes personal care and assistance, companionship and socialisation, home safety, assistance with medication, home-cooked meals, transport and appointments, shopping and light housekeeping, fall prevention exercises, and wound care.
